Direct Answer — North Carolina · 2026
In North Carolina, you must sign up for Medicare during your Initial Enrollment Period (IEP) — a 7-month window that starts 3 months before your 65th birthday month and ends 3 months after. If your birthday is in June 2026, your IEP runs March 1 through September 30, 2026. Sign up at SSA.gov or call Social Security at 1-800-772-1213. People already drawing Social Security at 65 are auto-enrolled in Parts A and B; everyone else must enroll themselves. If you delay Part B without creditable employer coverage, you owe a permanent 10% penalty for every full 12 months delayed — that's $20.29/month extra on top of the standard $202.90 Part B premium for the rest of your life. What are people really worried about with Medicare timing?

The four questions NC residents ask me most often when they're staring down their 65th birthday.

01
What if I miss my IEP — how bad is the penalty?
10% per full 12-month delay, added to your $202.90 monthly Part B premium permanently. A 2-year delay means $40.58/month extra forever — over $9,700 across 20 years of Medicare.
02
Will I have a coverage gap if I time the transition wrong?
If you sign up in the last 3 months of your IEP, Part B coverage starts the month after enrollment — not your birthday month. That can leave a 1-3 month gap with no medical coverage.
03
Do I really need Part B if I have employer coverage?
Only if your employer has 20+ employees AND coverage is creditable. Under 20 employees, Medicare is primary at 65 — the employer plan pays second and the penalty clock is running even if you keep both.
04
When do I have to pick Medicare Advantage vs Medigap?
Your Medigap Open Enrollment is a one-time 6-month window starting the month your Part B is effective. Miss it and you can be denied Medigap or charged more for pre-existing conditions in NC.
⚠️ The penalty is permanent — not a one-time fee

Many NC residents think the late enrollment penalty is a fine they pay once. It's not. The 10% surcharge gets added to your Part B premium for as long as you have Medicare. Delay 1 year = $20.29/mo extra for life. Delay 3 years = $60.87/mo extra for life ($14,608 over 20 years). Delay 5 years = $101.45/mo extra for life. There is no cap and no statute of limitations.

💡 Broker Tip — North Carolina

The single most common mistake I see in NC is folks turning 65 who are still working at small companies (under 20 employees) — restaurants, family practices, contractors, churches. They keep the employer plan thinking they're protected. They're not. Medicare is primary at 65 when the employer has fewer than 20 employees, which means the employer plan only pays as a secondary — and the Part B penalty clock starts ticking the moment your IEP closes. Before you assume you can delay, ask your HR or owner: "How many full-time-equivalent employees do we have?" If the answer is 19 or fewer, enroll in Part B during your IEP. No exceptions.

What does delaying Part B actually cost over 20 years?

Delayed 2 years
+$9,739
A Wake County retiree who delays Part B for 2 full years owes a permanent 20% penalty — $40.58/month on top of the standard premium. Over 20 years of Medicare, that's $9,739 in penalties alone, before any premium increases.
Enrolled on time
$0 penalty
A Durham County resident who enrolls during her IEP pays only the standard $202.90/month Part B premium. No penalties, no surcharges, full Medigap underwriting protection during the 6-month open enrollment window.

Why doesn’t it cost you anything to work with a broker?

It doesn’t — because I get paid by the insurance carrier to manage your plan. Most call centers get paid more to steer your business toward certain carriers based on volume and contracts. The only thing I’m optimizing for is making sure you’re covered correctly when you actually need it. That’s what keeps people coming back. And referring their neighbors.

Frequently asked questions
What happens if I miss my Initial Enrollment Period in NC?
You face a 10% Part B penalty for each full 12-month period you delayed enrollment, added permanently to your $202.90 monthly Part B premium. You can only enroll during the General Enrollment Period (January 1–March 31) with coverage starting the first of the month after enrollment. A 2-year delay means $40.58/month extra for the rest of your life — over $9,700 in penalties across 20 years of Medicare.
Can I delay Medicare past 65 if I’m still working?
Yes, but only if your employer has 20 or more employees and you have creditable coverage. You then qualify for an 8-month Special Enrollment Period that begins the month after your employment or group coverage ends. If your employer has fewer than 20 employees, Medicare becomes primary at 65 and you must enroll in Part B on time or face the penalty — even if you keep the employer plan. See our working past 65 guide for details.
When is the 2026 Medicare Open Enrollment Period?
Annual Open Enrollment runs October 15 through December 7, 2025, for coverage starting January 1, 2026. This window is for switching Medicare Advantage plans, joining or dropping Part D, or moving between Medicare Advantage and Original Medicare. It does not apply to first-time enrollment in Parts A and B — that’s your IEP. Special Enrollment Periods exist for life events like moving or losing coverage; see our SEP guide.
How much does Medicare cost in NC in 2026?
Part B costs $202.90 per month with a $283 annual deductible. Part A is premium-free if you or your spouse worked 10+ years (40 quarters) paying Medicare taxes — otherwise up to $518/month. The Part A hospital deductible is $1,736 per benefit period. Medicare Advantage in-network out-of-pocket maximum is capped at $9,350. Part D drug spending is capped at $2,100 per year and insulin costs $35/month maximum. If your income is below 150% of federal poverty, see our Extra Help/LIS guide.
What’s the most common Medicare timing mistake NC residents make?
Assuming Medicare is automatic at 65. Only people already drawing Social Security at 65 are auto-enrolled in Parts A and B. Everyone else must actively sign up at SSA.gov or by calling Social Security. The second most common mistake is keeping a small-employer plan past 65 thinking it’s creditable — if the employer has fewer than 20 employees, Medicare is primary and the penalty clock is running. Call (828) 761-3326 if you’re unsure about your employer size or creditable coverage status.
