What Are Medicare Savings Programs?
Medicare Savings Programs (MSPs) are state-run Medicaid programs that help North Carolina residents with limited income pay for Medicare costs. NC operates three programs β QMB, SLMB, and QI β with different income limits and different levels of help. Critically, all three programs automatically qualify you for full Extra Help with Part D drug costs, without a separate application. A licensed broker can check your eligibility for free in about 10 minutes.
MSPs are separate from your Medicare plan. They're financial assistance that pays some or all of your Medicare costs, regardless of whether you have Original Medicare, Medicare Advantage, or a standalone Part D plan. In North Carolina, applications are processed through your county Department of Social Services (DSS) β not through Medicare.gov or Social Security directly.
The biggest obstacle isn't eligibility β it's awareness. Most eligible people never apply because they assume their home, car, or modest savings will disqualify them. They won't. If you have income under $1,816/month and countable resources under $9,950, you may qualify for at least QI β that's $202.90 per month in Part B premium savings alone.

QMB, SLMB, and QI: Full 2026 Comparison
Each program has different income limits and covers different costs. Here's the complete breakdown for 2026:

If you qualify for QMB, Medicare providers are legally prohibited from billing you for deductibles, copays, or coinsurance on Medicare-covered services. What Counts as "Resources" β and What Doesn't
The resource limit for all three programs in 2026 is $9,950 for individuals and $14,910 for couples. Many common assets are completely excluded from this calculation β which is why many people who assume they're ineligible actually do qualify.

If your countable resources are close to the limit β say $10,500 β you should still apply. First, the limits sometimes adjust. Second, I've seen people get approved because assets they thought were countable actually weren't in their specific situation. The worst that happens is you get a "not eligible" notice and try again next year when circumstances change. Don't self-disqualify. Apply and let the DSS determine eligibility.
βHereβs what Medicare Advantage actually costs when something goes wrong.β
Your PCP visit is $0. Your blood work is $0. Then you have a cardiac event. A cancer diagnosis. A surgery that requires a specialist who isnβt in your network. Now youβre looking at an $8,300 out-of-pocket maximum, prior authorization delays, and a facility bill you didnβt expect. The $0 premium plan isnβt free β youβll find that out the hard way, or you wonβt.
How to Apply for MSP in North Carolina
MSP applications in NC are processed through your county Department of Social Services (DSS) β not through Medicare.gov, the Social Security Administration website, or Medicare.gov directly. Here's the complete process:
Gather your documents
You'll need: a Social Security award letter or recent income statement, recent bank and investment account statements, your Medicare card, and a valid photo ID. If you have a pension, bring that statement too.
Contact your county DSS
Apply in person at your county Department of Social Services, by mail, or by phone. You can find your county DSS at ncdhhs.gov or call the NC Medicaid Helpline at 1-800-541-2831. Most NC counties also allow online applications through the ePASS portal.
Wait for determination
Processing takes up to 45 days for QMB. SLMB and QI benefits may be retroactive up to 3 months before your application date β meaning you could get reimbursed for Part B premiums you already paid.
Reapply each year
All three programs require annual redetermination. QI applicants must reapply each year, and enrollment is first-come, first-served with priority going to prior QI recipients. Apply early in the calendar year β typically January or February β to secure your spot before funding runs out.
QI is funded differently from QMB and SLMB. Enrollment is capped and applications are prioritized in order of receipt, with priority going to those who received QI the previous year. If you received QI last year, reapply at the start of the new calendar year. Don't wait until fall β funding can run out.
MSPs and Extra Help β The Double Benefit
This is the single most important fact about Medicare Savings Programs: qualifying for any MSP automatically qualifies you for full Extra Help (Low Income Subsidy) with Part D drug costs. You do not need to apply separately through Social Security.
Full Extra Help in 2026 eliminates Part D premiums, eliminates the Part D deductible, and reduces drug copays to $5.10 for generics and $12.65 for brand-name drugs. There is no late-enrollment penalty and no coverage gap.

Even if your income is too high for any MSP, you may still qualify for partial Extra Help through Social Security. The Extra Help income limit is higher β approximately $22,590/year individual or $30,660/year couple in 2026. Partial Extra Help still reduces drug costs significantly, even if it doesn't eliminate them entirely. Don't assume you're over the limit without checking both programs separately.

MSPs Work With Any Medicare Plan Type
Medicare Savings Programs are financial assistance β not a plan type. They work alongside whatever Medicare coverage you currently have:
If you qualify for QMB, the cost equation for Medigap vs. Medicare Advantage changes significantly. QMB already eliminates most Medicare cost-sharing β so paying $150β$300/month for a Medigap plan may not make financial sense. Conversely, with QMB, a Medicare Advantage plan with a $0 premium may give you extra benefits (dental, vision, OTC) on top of your existing protections. Common Mistakes to Avoid
Assuming you don't qualify without checking
Your home and car don't count as resources. Earned income is only partially counted. If you have income under $1,816/month and countable assets under $9,950, you may qualify for at least QI. The only way to know for certain is to apply β or let us do a free eligibility check over the phone.
Not reapplying for QI on time
QI requires annual reapplication and is first-come, first-served. If you received QI last year and don't reapply early in the new calendar year, you can lose benefits for the entire year. Set a reminder each January.
Not applying for Extra Help separately if denied MSP
Even if your income is too high for MSP, you may still qualify for Extra Help (LIS) through Social Security. The Extra Help income limit in 2026 is approximately $22,590/year for individuals β meaningfully higher than QI. Partial Extra Help still reduces drug costs significantly.
Paying bills you don't owe on QMB
If you have QMB and receive a bill for Medicare-covered services, you do not have to pay it. Providers are legally prohibited from balance billing QMB beneficiaries. If you're being billed, contact your State Health Insurance Assistance Program (SHIIP at 1-855-408-1212) or call us.
Not checking both MSP and Extra Help together
MSP and Extra Help are separate programs that can stack. Applying for one doesn't automatically get you the other β but being approved for any MSP does automatically qualify you for full Extra Help. A licensed broker can screen for both in a single 10-minute conversation.
