Beyond the pricing method, several factors affect what you'll actually pay:
- Age: Under attained-age pricing (most NC carriers), your premium increases each year. A Plan G that's $125/month at 65 might be $175/month at 75.
- ZIP code: Rates vary within NC. Urban areas with higher healthcare costs (Triangle, Charlotte) often have higher premiums than rural areas.

What a Medigap Quote Tells You (and What It Doesn't)
A Medigap quote shows one number: your monthly premium. Because benefits are standardized, that's the only variable between carriers for the same plan letter. But a smart comparison goes deeper:
- Rate increase history: How much has this carrier raised rates in the past 3-5 years? A carrier with 4-6% annual increases is more stable than one with 12-15% spikes.
- AM Best rating: Financial strength matters. You want a carrier rated A or better β meaning they can pay claims long-term.
- Available discounts: Household, non-smoker, EFT (electronic payment), and annual-pay discounts can reduce premiums 5-15%.
- Claims experience: How easy is it to file and resolve claims? Some carriers have better reputations for customer service.

NC Disability Rule: Under-65 Medigap Access
North Carolina law (G.S. 58-54-45) guarantees that individuals under 65 who qualify for Medicare due to disability can purchase Medigap Plans A, D, or G. Premiums may be higher than for beneficiaries 65+, and a pre-existing condition waiting period of up to 6 months may apply. When you turn 65, you get a new 6-month open enrollment period to purchase any Medigap plan at standard rates.
Medigap + Part D: The Full Picture
Medigap does not include prescription drug coverage. You'll need a separate stand-alone Part D plan. Your total monthly cost with Medigap is:

When to Get Quotes
- Turning 65: Start comparing 2-3 months before your 65th birthday. You can apply up to 60 days early. Your 6-month guaranteed-issue window starts the month you turn 65 and have Part B.
- Losing employer coverage after 65: Some carriers offer guaranteed issue rights if you're leaving an employer plan. Timing is critical β apply immediately.
