What Is Extra Help (Low-Income Subsidy)?
Extra Help — also called LIS (Low-Income Subsidy) — is a federal program that pays part or all of your Medicare Part D prescription drug costs. It can reduce or eliminate your Part D premium, deductible, and copays at the pharmacy. The program is administered by Social Security and available in every state, including North Carolina.

Eligibility is based on income and resources (savings, investments, real estate other than your home). For 2026, approximate limits are:
Individual
Married Couple
Limits are approximate and updated annually by CMS. Some people with income slightly above these limits may qualify for partial Extra Help. Apply even if you're close — the worst outcome is "not eligible," and you can reapply if your income changes.
What Extra Help Covers
If you qualify for full Extra Help, the program can:
- Eliminate your Part D monthly premium (or reduce it to $0)
- Eliminate the Part D deductible (normally up to $615 in 2026)
- Reduce copays to $0–$4.50 for generics and $0–$11.20 for brand-name drugs
- Eliminate the late-enrollment penalty if you qualify for full LIS
If you qualify for partial Extra Help, you still get reduced premiums, a lower deductible, and reduced copays — just not as much as full LIS.
LIS only reduces Part D drug costs. It does not pay your Part B premium, hospital deductibles, or doctor copays. How to Apply for Extra Help in North Carolina
There are four ways to apply:
- Online: Apply at ssa.gov/extrahelp — the fastest option
- By phone: Call Social Security at 1-800-772-1213 (TTY 1-800-325-0778)
- In person: Visit your local Social Security office
- Through your county DSS: NC county Departments of Social Services can also process applications
You'll need information about your income, savings, investments, and living expenses. You do not need to provide your Social Security Number just to check eligibility or compare plans — only when you're ready to formally apply.
Who Gets Enrolled Automatically?
You don't need to apply if you already receive:
- Medicaid (full benefits)
- Supplemental Security Income (SSI)
- A Medicare Savings Program (QMB, SLMB, or QI)
If you receive any of these, you're automatically enrolled in full Extra Help. Social Security will notify you by mail.
How Extra Help Works With Your Drug Plan
Extra Help is not a drug plan itself — it's financial assistance that works on top of whatever Part D plan you have. It applies to:
- Standalone Part D plans — used with Original Medicare or Medigap
- Medicare Advantage plans with drug coverage (MA-PD) — the drug portion gets LIS assistance
You still need to choose a Part D or MA-PD plan that covers your specific medications. Extra Help reduces what you pay on that plan — but it doesn't change which drugs are covered. That's why matching your medication list to the right formulary still matters, even with LIS.

If you qualify for a Medicare Savings Program, you automatically qualify for full Extra Help. Apply for both — many NC beneficiaries are eligible for both and don't realize it.
Common Mistakes With Extra Help
- Assuming you don't qualify: The income limits are higher than most people expect. Apply even if you're close.
- Not reapplying after income changes: If you were denied previously but your income has dropped (retirement, spouse's death, job loss), reapply.
- Keeping the wrong drug plan: LIS reduces costs on whatever plan you have — but if your plan's formulary doesn't cover your drugs well, you're still overpaying. Review your plan every fall during AEP.
- Ignoring Medicare Savings Programs: If you qualify for LIS, you may also qualify for MSP help with Part B premiums. Check both.
