Why You Can't Do This Yourself
1. Medicare.gov's plan finder doesn't know which Duke Health doctors accept Medicare Advantage
The website says "Duke Health in-network" but Duke University Hospital and Duke Raleigh Hospital are often on different contracts. One might be in-network, the other requires a $500/day copay.
2. Insurance company websites list doctors who stopped taking MAPD patients 6 months ago
Duke Health specialists can be "in-network" on paper but won't accept Medicare Advantage referrals. You won't find out until you show up for your appointment.
3. Drug formularies change mid-year and your $15/month medication becomes $180/month
Plans can move drugs between tiers. If you're on 4+ medications, you need someone checking this every year, not just at enrollment.
4. "In-network" doesn't mean what you think it means
Your Duke Health cardiologist might be in-network, but if Heart Care (Duke Heart Center) requires prior authorization and your plan denies it, you're paying $12,000 out-of-pocket for the procedure.

What Actually Happens If You Pick Wrong
Real scenario from 2025:
Client A picked Humana Gold Plus because $0 premium sounded good.
Then she was diagnosed with breast cancer.
Her Duke Health oncologist was in-network. Duke University Hospital was in-network.
But Cancer Care (Duke Cancer Institute) radiation was NOT in-network.
She found out after her first radiation appointment when the bill came:
$38,000 out-of-pocket maximum vs the $2,500 she expected.
We switched her to Aetna Medicare Eagle during a Special Enrollment Period (cancer diagnosis qualifies).
Cancer Care (Duke Cancer Institute) radiation was in-network. She paid $2,500 total for the year.
The difference between picking the right plan and picking the wrong plan was $35,500.
