Chapel Hill and Hillsborough are served by UNC Health — UNC Medical Center, UNC Hillsborough and the UNC Physicians Network — with Duke Health in neighbouring Durham taking a meaningful share of Orange County residents.

The two academic systems are ten miles apart and contract separately. Orange County residents frequently use both, and a plan that covers UNC fully may treat Duke as out-of-network entirely despite the distance being trivial.

Choosing a Medicare Advantage plan that excludes UNC Hospitals from its specialist tier.
Why it hurtsSome MA plans technically include UNC Hospitals as in-network for primary care but require a specialist referral that UNC won’t honor without an in-system PCP. Chapel Hill and Carrboro residents end up driving to Burlington or Raleigh for specialists they could have seen 5 minutes from home.
What to doVerify both PCP and specialist tier with the carrier directly. A PPO or Medigap Plan G/N keeps the full UNC Health system open to you without referral roadblocks.

Mistake 02 Orange-specific
Not realizing UNC Lineberger Comprehensive Cancer Center has stricter network rules than community oncology.
Why it hurtsLineberger is one of only 3 NCI-designated comprehensive cancer centers in NC, and access to clinical trials matters if you’re facing a serious diagnosis. Several Orange County MA plans cover UNC oncology but exclude Lineberger’s research-track programs. By the time you find out, you’re mid-treatment.
What to doIf cancer history runs in your family, prioritize Medigap or a PPO that puts Lineberger’s full research access in-network. Don’t wait until diagnosis to fix it — Medigap underwriting can lock you out later.

Mistake 03 Orange-specific
Picking a plan based on the Hillsborough campus and discovering most specialists are on the Chapel Hill main campus.
Why it hurtsThe UNC Hospitals Hillsborough Campus handles a focused set of services. A lot of specialty care still happens at the main Chapel Hill campus. If your plan’s “preferred” UNC site is Hillsborough only, you’ll pay a higher copay every time you cross 86 to Chapel Hill.
What to doConfirm both campuses are at the same in-network tier before enrolling. For Hillsborough residents, this is the single most common Medicare mistake I fix during AEP reviews.

Mistake 04 NC-wide
Missing the Part B late enrollment penalty if you’re working past 65.
Why it hurtsIf you don’t sign up for Part B during your Initial Enrollment Period and don’t have qualifying employer coverage, you owe a 10% penalty for every 12 months you delayed — for the rest of your life. A 3-year delay = 30% higher Part B premium forever.
What to doIf you’re working past 65, get a Certification of Creditable Coverage from HR before you stop work. That document protects you from the penalty.

Mistake 05 NC-wide
Missing your 6-month Medigap open enrollment window.
Why it hurtsYou have one 6-month window after you turn 65 and enroll in Part B to buy any Medigap plan with no medical underwriting. Miss it and NC insurers can deny you coverage — or charge dramatically more — if you develop a health condition. This is the single most expensive mistake I prevent.

If you delay Medicare Part B without creditable employer coverage, you’ll pay a 10% lifetime penalty for every 12 months you delayed — added to your Part B premium for the rest of your life. The 2026 standard Part B premium is $202.90/month, so a 24-month delay raises it to roughly $243/month, permanently. Your Initial Enrollment Period is a 7-month window: 3 months before your 65th birthday month, the birthday month itself, and 3 months after.

Before you enroll in any Medicare Advantage plan, give me the names of every doctor you see — primary care, cardiologist, oncologist, endocrinologist — and I’ll call each one’s billing office directly to verify they accept the plan. Provider directories on Medicare.gov run 4–6 months stale, and “in-network” on the directory doesn’t always mean the provider is accepting new patients on that plan.
