Medicare Extra Help (Low Income Subsidy / LIS) reduces or eliminates Part D premiums, deductibles, and copays for NC residents with limited income and resources. In 2026, the income limit is $23,475/yr for an individual and $31,725 for a couple. Resource limits are $17,600 single / $35,130 married — excluding your home, one car, and life insurance. Full Extra Help saves NC seniors $3,900–$5,900 per year on prescription drug costs. Apply free at ssa.gov or call Robert Simm at (828) 761-3326 for a free eligibility check.
The broker's answer
Here's what I see every week in North Carolina: someone walks in paying $400 a month for prescriptions and assumes that's just what Medicare costs. They've never heard of Extra Help. Or they heard about it, looked at the income limits, counted their house and car as assets, and decided they didn't qualify. They were wrong — your house and car don't count. I run a 15-minute eligibility check for every client. About 1 in 4 NC seniors I talk to either qualifies outright or is close enough that we should apply. The program saves $4,000–$6,000 a year. That's not a rounding error — that's grocery money, that's keeping the heat on. If you're on Medicare in North Carolina and you haven't checked, you should.

⚠ Extra Help mistakes I correct every month
Mistake 01 NC-wide
Counting assets that don't count — and never applying.
Why it hurtsNC seniors look at their house, car, and savings and assume they're over the resource limit. But the SSA excludes your primary residence, one vehicle, life insurance under $1,500, irrevocable burial contracts, and Social Security back payments. A homeowner with $15,000 in savings qualifies. Most people never find out because they self-screen incorrectly.
What to doApply anyway. The application is free and there's no penalty for being denied. Or call (828) 761-3326 for a 15-minute eligibility check — I can tell you in one conversation whether it's worth applying.
Mistake 02 NC-wide
Staying on the CMS auto-assigned Part D plan after getting Extra Help.
Why it hurtsWhen CMS approves your Extra Help, it may auto-assign you to a benchmark Part D plan. That plan may not cover your specific medications at the best tier. You could be paying $11.20 for a brand-name drug that another plan covers as a $4.50 generic. Over 12 months, the wrong auto-assignment costs $300–$800 even with Extra Help.
What to doUse your Special Enrollment Period — Extra Help recipients can switch Part D plans once per calendar quarter, any time of year. Have a broker run your drug list against every available plan in your NC county.
Mistake 03 NC-wide
Not knowing Extra Help gives you a year-round Special Enrollment Period.
Why it hurtsMost Medicare beneficiaries can only change plans during AEP (Oct 15–Dec 7). But Extra Help recipients get a continuous SEP — you can switch plans once per quarter, all year. NC seniors who don't know this stay locked into bad plans for 12 months when they could switch immediately.
What to doIf you have Extra Help and your prescriptions change — new drug, new dose, new diagnosis — call your broker. You can switch plans right away, not next October.
Mistake 04 NC-wide
Not applying for the Medicare Savings Program at the same time.
Why it hurtsExtra Help covers Part D drug costs. The Medicare Savings Program (MSP) covers Part A/B premiums, deductibles, and copays — and MSP automatically qualifies you for Extra Help. Many NC seniors apply for one but not the other, leaving thousands in additional savings on the table.
What to doApply for MSP through your county Department of Social Services (NC DHHS). If approved for QMB, SLMB, or QI, you get Extra Help automatically — no separate SSA application needed. Read the NC Medicare Savings Programs guide →
Mistake 05 NC-wide
Filling prescriptions at an out-of-network pharmacy.
Why it hurtsExtra Help copays ($4.50 generic / $11.20 brand) only apply at pharmacies within your Part D plan's network. Using an out-of-network pharmacy — even one down the street — means you pay full price. Some NC rural counties have limited pharmacy networks, and the nearest in-network pharmacy may be 20+ miles away.
What to doBefore enrolling in any Part D plan, verify that your preferred pharmacy is in-network. I check pharmacy networks for every NC client — including chains and local independents — before recommending a plan.

Auto-qualifies for other?No — doesn't auto-qualify for MSPYes — MSP auto-qualifies you for Extra Help
Special Enrollment Period?Yes — switch plans quarterlyNo — but MSP triggers Extra Help SEP
Can you have both?Yes — many NC seniors qualify for both and should apply for both
⚠ Don't skip doses to save money
If you're rationing medications because of cost, Extra Help may cover most or all of your drug expenses. Full Extra Help caps generics at $4.50 and brand-names at $11.20 — per prescription, not per month. Many NC seniors on fixed incomes spend $300–$600/month on drugs that Extra Help would reduce to under $50/month total. The application is free and takes 15 minutes at ssa.gov/extrahelp.
💡 Broker Tip · Apply for both programs
Extra Help covers your drugs. The Medicare Savings Program covers your Part B premium ($202.90/month in 2026). If you qualify for MSP, you automatically get Extra Help — no separate application to Social Security needed. Apply for MSP at your county Department of Social Services. I help NC clients navigate both applications at no cost.

Frequently asked questions about Extra Help in NC

What is the difference between Extra Help and the Medicare Savings Program?
Extra Help (LIS) specifically reduces your Medicare Part D prescription drug costs — premiums, deductibles, and copays. The Medicare Savings Program (MSP) is a separate North Carolina program that helps pay your Medicare Part A and Part B costs, including the $202.90 Part B monthly premium. Many people qualify for both simultaneously. Qualifying for MSP often automatically qualifies you for Extra Help as well. Read the NC Medicare Savings Programs guide →
What assets are excluded from Extra Help resource limits?
The SSA excludes your primary residence, one vehicle, personal belongings, household goods, life insurance policies with a combined face value of $1,500 or less, irrevocable burial contracts, and any back payments from Social Security or SSI. For married couples, these exclusions apply jointly. This is why many NC seniors who think they have "too many assets" actually qualify.
Can I get Extra Help if I already have Medicare Advantage?
Yes. Extra Help works with Medicare Advantage Prescription Drug plans (MA-PD) as well as standalone Part D plans. If you have Extra Help and enroll in a Medicare Advantage plan with drug coverage, your drug cost-sharing is reduced the same way. Extra Help recipients also get a Special Enrollment Period to switch plans quarterly.
How do I apply for Extra Help in North Carolina?
Three ways: (1) Online at ssa.gov/extrahelp — takes about 15 minutes; (2) By phone at 1-800-772-1213; (3) In person at your local Social Security office. You can also apply through NC DHHS for the Medicare Savings Program, which may qualify you for Extra Help automatically. Call Rob Simm at (828) 761-3326 for help preparing your application.
Is it too late to apply if I'm already on Medicare?
No. You can apply for Extra Help at any time — there is no enrollment window. If approved, your benefit typically begins the first day of the month following approval. You can't receive retroactive payments for prior years, but you can start saving immediately.
How long does it take to get approved?
The SSA typically processes applications within 30 days. Online applications may receive faster responses. Once approved, your benefit usually begins the first day of the month following your approval. If you're auto-enrolled through Medicaid or SSI, enrollment is immediate.
What happens if my income changes after I'm approved?
The SSA conducts annual redeterminations. If your income rises above 150% FPL, you may receive partial Extra Help or lose the benefit. You'll receive a written notice and have 60 days to appeal. Rob Simm tracks annual redeterminations for all NC clients to ensure no one loses their benefit unexpectedly.
Does this answer vary by NC county?
Extra Help income and resource limits are federal and don't vary by county. However, the Part D plans available to you — and which ones best match your specific medications — vary significantly by NC county and ZIP code. A county-specific drug plan comparison ensures Extra Help delivers maximum savings on your actual prescriptions.

Does it matter which Medicare carrier you choose?

It doesn't — because I get paid by the insurance carrier to manage your plan. Most call centers get paid more to steer your business toward certain carriers based on volume and contracts. The only thing I'm optimizing for is making sure you're covered correctly when you actually need it. That's what keeps people coming back. And referring their neighbors.
