Tier 1: NC Medicaid

Who qualifies: Income under ~$20,783 single / ~$42,972 family of 4. NC expanded Medicaid in Dec 2023 β€” hundreds of thousands newly eligible. What you get: $0 premium, minimal copays, comprehensive coverage including doctors, hospitals, prescriptions, mental health. Enroll: Year-round, no waiting for Open Enrollment. Apply at HealthCare.gov or NC DHHS.
Tier 2: ACA Silver + Cost-Sharing Reductions
$0–$150/mo
Who qualifies: Income ~$20,783–$36,450 single (100%–250% FPL). What you get: Subsidized Silver plan with CSR β€” lower deductible (~$500–$2,500 instead of $6,000), lower copays, lower OOP max. At $25K income: ~$50–$100/month. At $30K: ~$100–$150. Why Silver specifically: CSR only applies to Silver plans. Picking Bronze to save $20/month means losing thousands in cost-sharing benefits.
Tier 3: ACA Silver/Gold With Subsidies (No CSR)
$150–$400/mo
Who qualifies: Income ~$36,450–$60,240 single (250%–400% FPL). Still get premium tax credits but no CSR. Silver at $40K: ~$150–$250/month. Gold at $50K: ~$300–$400/month. Gold has lower deductible ($1,000–$2,500) but higher premium. Best choice depends on: How often you use care. Regular specialist visits or prescriptions = Gold may save more in total annual cost.
Tier 4: COBRA

What it is: Continue your former employer's plan at full cost (employer share + your share). No subsidies. Typical cost: $600–$800+/month individual. Feels free β€” until it isn't. One ER visit: $2,000–$5,000. One hospital stay: $10,000–$50,000+. One surgery: $30,000–$100,000+. Even healthy 25-year-olds: One car accident or appendicitis and you're looking at medical bankruptcy.
