Which ACA Marketplace Plans in Wake County Cover Mental Health Therapy With Low Copays?
Every ACA plan covers therapy by law. But copays range from $0 to $75 per session. The plan tier and your income determine whether therapy is affordable or just “covered.”
Answered by Robert Simm, a Licensed Health Insurance Broker in North Carolina.
I price the subsidy against your actual income, not last year’s. Healthcare.gov shows you what plans cost — it can’t tell you whether you’re about to cross the cliff that took subsidies to zero in 2026.
What you’ll actually pay per therapy session in Wake County
| Plan tier | Therapy copay | Annual cost (weekly) | Notes |
|---|---|---|---|
| Bronze | Full price until deductible, then $40–$75 | $7,000+ (deductible) + copays | Worst tier for regular therapy. You pay $150–$250/session until $7K+ deductible met. |
| Silver (no CSR) | $30–$50 after deductible | $4,000+ deductible + ~$2,000 copays | Better than Bronze but deductible still applies to therapy on most plans. |
| CSR Silver (150% FPL) | $0–$10/session | $0–$520/year | Best value. Deductible as low as $0. Therapy copays kick in immediately. |
| CSR Silver (200% FPL) | $10–$25/session | $520–$1,300/year | Strong value. Low deductible means copays start fast. |
| Gold | $20–$40/session | $1,040–$2,080/year | No deductible for many services. Predictable copays. Higher premium. |
Copay ranges are illustrative for 2026 Wake County plans. Actual copays vary by carrier and specific plan. "Therapy" includes outpatient visits with psychologists, psychiatrists, LCSWs, and LPCs. Mental Health Parity requires therapy copays to be no higher than comparable medical visit copays on the same plan.

What I check for therapy coverage
Which WakeMed facilities serve Wake County?
WakeMed is the health system on record for Wake County, and 3 facilities in the county carry its name — which does not mean they all sit on the same plan contracts.
- WakeMed Raleigh Campus
- WakeMed Cary Hospital
- WakeMed North Hospital
WAKEMED NETWORK NOTES
WakeMed went out-of-network with all UnitedHealthcare plans on 15 November 2025, commercial and Medicare Advantage alike, after the two sides could not agree on rates. WakeMed hospitals, outpatient clinics, 210 PET, urgent cares and physician specialists are all out of network. Verified .
The carve-out is the part people get wrong. WakeMed's primary care physicians and MyCare365 sit on a separate contract and did not go out-of-network — but physician specialists did. So a Wake County patient on a UnitedHealthcare Medicare Advantage plan can keep seeing their WakeMed primary care doctor in-network while every WakeMed hospital admission, ER visit, imaging appointment and specialist visit bills at out-of-network rates. Keeping your doctor is not the same as keeping your hospital, and the hospital is where the money is. Verified .
This is not isolated to Raleigh. UnitedHealthcare is reducing its Medicare Advantage footprint across 109 counties in 16 states for 2026. A UnitedHealthcare plan that worked in Wake County in 2025 is worth re-checking for 2026 whether or not you have heard from the carrier. Verified .
Frequently asked questions
Do ACA plans limit the number of therapy sessions per year?
Are psychiatry visits covered the same as therapy?
What about mental health medications?
Can I switch plans if my therapist isn’t in-network?
Is therapy covered if I’m on Medicaid?
Why does mental health coverage need a broker?
Because “mental health services covered” on a plan summary doesn’t tell you whether your therapist is in-network, whether therapy copays apply before or after the deductible, or whether your antidepressant is on Tier 1 or Tier 3. HealthCare.gov can’t answer those questions. I verify your specific therapist, check your medications, compare copay structures across every plan in Wake County, and find the one where weekly therapy is actually affordable — not just “covered.”
