What is Medicare Extra Help?
Medicare Extra Help — also called the Low-Income Subsidy, or LIS — is a federal program that helps people with limited income and assets pay for their Medicare Part D prescription drug coverage. It’s administered jointly by the Social Security Administration and the Centers for Medicare & Medicaid Services.
If you qualify, Extra Help reduces or eliminates three costs that make Part D expensive: your monthly premium, your annual deductible, and your copays at the pharmacy. In 2026, Extra Help recipients pay no more than $5.10 for each generic drug and $12.65 for each brand-name drug. Once your total out-of-pocket drug costs reach $2,100, your covered medications become completely free for the rest of the year.
Extra Help also eliminates any Part D late enrollment penalty you might owe — a benefit that can save you hundreds of dollars annually if you delayed signing up for drug coverage when you first became eligible.

Who qualifies for Extra Help in Wake County?
Eligibility is based on three factors: you must be enrolled in Medicare, your income must be below certain limits, and your countable assets must be below certain limits. Wake County doesn’t have different eligibility rules than the rest of North Carolina — the federal limits apply — but the Part D plans available to you (and their premiums for Extra Help recipients) are specific to your county.

Important: your home, your car, household belongings, and burial plots are not counted as assets. Retroactive Social Security or SSI payments are also excluded for nine months after you receive them.
Social Security uses countable income, not gross income. Food stamps (SNAP), housing assistance, home energy assistance, and certain other benefits are not counted. Neither are retroactive Social Security payments for 9 months after receipt. If your income is within $2,000–$3,000 of the limit, you should absolutely apply — the exclusions may bring you under.
How do I apply for Extra Help in Wake County?
There are three ways to apply, and there is no enrollment window — you can apply any time during the year. If approved, you’ll also receive a Special Enrollment Period that lets you switch Part D plans once per month, so you can move to the best plan for your medications right away.
What documents do I need?
Before applying, gather your most recent Social Security benefits award letter, bank and investment account statements, pension or annuity statements, and your Medicare card number. If you’re married and living together, you’ll need this information for both spouses. Don’t let missing documents stop you — Social Security will accept your best estimates if you don’t have exact figures.
What happens after I apply?
Social Security will review your application and send you a decision letter, usually within a few weeks. If approved, you’ll be automatically enrolled in a Part D plan if you don’t already have one. You can then choose a different plan if the one assigned to you doesn’t cover your medications well — and this is exactly where Rob can help.
