Turning 65
Your 7-month Initial Enrollment Period is the most important window in Medicare. Enroll in Part A and Part B, choose between Medicare Advantage or Medigap + Part D, and don't miss the 6-month Medigap Open Enrollment Period (guaranteed issue, no health questions). Working Past 65
If your employer has 20+ employees and you have creditable coverage, you can delay Part B without penalty. Enroll within 8 months of leaving the job or losing coverage. Critical traps: COBRA is NOT creditable for Part B (your 8-month window starts when you leave the job, not when COBRA ends). Small employers (under 20): Medicare becomes primary β enroll in Part B at 65.

Missed Your Enrollment Window
If you missed your IEP without qualifying employer coverage, you face permanent penalties and may need to wait. The General Enrollment Period (January 1 β March 31) lets you sign up for Part A and Part B, but coverage doesn't start until July 1 β leaving a gap. Penalties: Part B adds 10% of $202.90 per 12-month delay (permanent). Part D adds 1% of $38.99 per uncovered month (permanent).

Changing Plans During AEP (Oct 15 β Dec 7)
Every fall, you can switch MA plans, move between MA and Original Medicare, or change your Part D plan. Changes take effect January 1. We recommend reviewing every year β plans change formularies, networks, and premiums annually. In NC, some Part D plans saw $50/month premium increases from 2025 to 2026.

Moving to North Carolina
Moving triggers a Special Enrollment Period (typically 63 days). Medicare Advantage and Part D plans are county-specific β your current plan may not be available in your new NC county, and your doctors may not be in-network. You'll need to choose new coverage based on your new ZIP code.

Qualifying for a Special Enrollment Period
Beyond moving, SEPs are triggered by: losing employer coverage, losing Medicaid, leaving a Medicare Advantage plan (MA OEP), gaining Extra Help/LIS status, plan violations, and other qualifying events. Each has different timelines. Don't assume you don't qualify β many people have SEP rights they don't know about.

Low Income β MSPs and Extra Help
If your income is limited, Medicare Savings Programs (QMB, SLMB, QI) can pay your Part B premium and auto-qualify you for Extra Help with drug costs. Combined savings: $5,000β$8,000/year. Qualifying for these programs also opens Special Enrollment Periods. Income limits for 2026: up to $1,816/month individual for QI.
