ACA Marketplace (HealthCare.gov)
Under 65
Who: Self-employed, between jobs, early retirees, anyone without employer/Medicare/Medicaid coverage. Cost: Avg ~$450/month before subsidies. Many NC residents pay $0–$150/month after premium tax credits based on income. Silver plans with cost-sharing reductions (CSR) are often the best value. ACA OOP max: $9,200 individual / $18,400 family. Enroll: Open Enrollment Nov 1–Jan 15. Dec 15 deadline for Jan 1 start. Special Enrollment after qualifying life events. NC Medicaid (Expanded 2023)
Income-Based

Medicare (65+ or Disability)

Employer-Sponsored Coverage
Working
Who: Employees of companies offering group health insurance (and their dependents). Cost: Avg employee share ~$130/month individual, ~$530/month family (employer pays the rest). Often the most affordable option if available. Enroll: During employer's open enrollment period or within 30 days of hire/qualifying event. Note: If you're losing employer coverage (job change, retirement, layoff), you have a 60-day Special Enrollment Period for ACA Marketplace or Medicare.

The most common mistake I see: people turning 65 who assume Medicare replaces everything automatically. It doesn't. You still need to actively choose between Medicare Advantage and Medigap, enroll in Part D for drug coverage, and do it within your enrollment window β€” or face permanent penalties.
