Line these up for every plan you're considering

1. Total Annual Cost
Most Important

2. Doctor Network (by NPI)
Critical
Every doctor verified by NPI number β€” not just hospital name. Hospitalists, anesthesiologists, and specialists inside the same facility can have different network participation.
3. Drug Formulary + Tier

Your specific medications checked by name, dosage, and quantity. The same drug can be Tier 2 ($12/month) on one plan and Tier 3 ($47/month) on another. Also check step therapy and quantity limits.
4. Copay Schedule
