Part A β€” Hospital

Covers inpatient hospital stays, skilled nursing, hospice, some home health. Premium-free for most people with 40+ quarters of work history. $1,676 deductible per benefit period in 2026.
Part B β€” Medical
$202.90/mo
Covers doctor visits, outpatient care, lab work, preventive services, durable medical equipment. $283 annual deductible. Then Medicare pays 80%, you pay 20% β€” with no cap unless you add supplemental coverage.
Part C β€” Medicare Advantage
$0–$150/mo
Private plans replacing Original Medicare. Bundles A + B, usually includes Part D and extras (dental, vision, hearing). Network-based (HMO/PPO). OOP max up to $8,850. 175 plans in NC.
Part D β€” Prescription Drugs
$0–$90+/mo

Regardless of where you live in NC, Medicare begins with Part A and Part B β€” together called "Original Medicare." Part A covers hospitals. Part B covers doctors and outpatient care. Almost everyone enrolls in both.

⚠ The 20% Problem

Original Medicare Part B pays 80% of approved charges. You pay 20% β€” with no annual cap. That uncapped 20% is why Medigap or Medicare Advantage exists: both provide protection against catastrophic costs. Staying on Original Medicare alone leaves you financially exposed.

What Makes NC Different

Medicare is a federal program, but your plan options are local. Here's what varies by county in North Carolina:

  • Medicare Advantage plans: 175 statewide, but only 10–50 in any given county. Urban counties (Wake, Durham, Mecklenburg) have the most options. Rural counties have fewer but still include $0-premium plans.
  • Part D plans: 12 standalone plans statewide. Down from 14 in 2025. Some saw $50/month premium increases.
  • Medigap plans: 10 standardized plans (A, B, C, D, F, G, K, L, M, N). Same benefits regardless of carrier β€” only price differs. NC uses attained-age pricing for most carriers.
  • Provider networks: Duke Health, UNC Health, Atrium, Cone, WakeMed, and Mission/HCA all have different participation across MA plans. Network verification by NPI number is essential.
